A detailed plan of care is developed based on the diagnosis, care needs and prognosis, family support, and home environment. A financial estimate is prepared for the proposed service ahead of time.

A team of health care and in-home support staff is selected and oriented to the case, and the total home care program is implemented. A HHC supervisor makes a periodic visit to the home to monitor treatment and review the care plan.
